It is critical that the screening and triage occur parallel to the physician's usual visit. It is unlikely that physicians would be able to take extra time in an already packed schedule to ask questions for a detailed mental health screen. This screening can be done in several ways, preferably via electronic forms, so that the results of a standard mental health screen are available at the time of a routine well child exam or the nursing personnel can initiate screening on a patient with chronic vague complaints or psychosocial presenting issues according to a protocol. Ideally, positive results on a screen could result in the nursing personnel initiating a referral to mental health triage, based on protocol. Some suggestions for screening instruments for the 0–4 years age group include the Ages and Stages Questionnaire (ASQ; Squires et al. 1998), Ages and Stages Questionnaire Socio-Emotional (ASQ-SE; Beligere et al. 2005), or the Pediatric Screening Checklists (PSC; Jellinek et al. 1999). Screening tools for the 5–12 years age group include the PSC, the Strength and Difficulty Questionnaire (SDQ; Glazebrook et al. 2003), or the Vanderbilt Behavioral Evaluation Tool (Williams et al. 2004). Adolescent screening tools include the PSC, the Patient Health Questionnaire—Adolescent Version (PHQ-A; Johnson et al. 2002), or the Vanderbilt Behavioral Evaluation Tool.
