Major depressive disorder is one of the most common medical disorders affecting adults in the world (Lopez and Murray 1998). In the United States the lifetime prevalence of major depression for men is 9% and 17% for women (Hasin et al. 2005), while the lifetime prevalence of dysthymia is 4% for men and 8% for women (Kessler et al. 1994). The lifetime prevalence of minor depression is 10% for individuals between the ages of 15 and 54 years (Kessler et al. 1997). For persons 65 years and older, the 1-month point prevalence of minor depression is 13%, which is twice the prevalence of major depressive disorder for this age group (Judd and Akiskal 2002; McCusker et al. 2005). The lifetime prevalence for subsyndromal depression is 11.8% for the general population (Goldney et al. 2004; Judd et al. 1994).
