Initiation: sodium, potassium, calcium, phosphate,
BUN, creatinine, TSH, T4, CBC, urinalysis, beta-HCG if appropriate;
ECG in patient older than 50 years or with preexisting cardiac disease
Maintenance: TSH, BUN/creatinine recommended every
6 months; ECGs as needed in patient older than 40 years or with preexisting
