This chapter will describe how to work with patients in constructing bio-psychosocial formulations for CBT. A general framework for developing CBT case conceptualizations has been outlined in an earlier book (Wright et al. 2006). This method is based on recommendations from the Academy of Cognitive Therapy (www.academyofct.org). Because details on basic procedures for case conceptualizations in CBT are available -elsewhere (Wright et al. 2006), we focus here on providing practical guidance on developing formulations for patients with severe mental disorders. Case examples, based on four of the individuals featured in the video illustrations, are used to demonstrate methods of developing formulations using a timeline and Socratic questioning—a type of case conceptualization produced jointly with the patient. This process often leads to an improvement in insight for the patient and guides the therapist in treatment planning.
