Questions Frequently Asked About the Problem-Otiented Record in Psychiatry

Abstract

With the aid of ward staff, the authors prepared answers to some questions most often asked about use of the problem-oriented medical record in psychiatry. The questions are presented in relation to the four components of the record: the data base, the problem list, the initial plan, and the progress notes.
