Excerpt

In Chapter 2 (“The Collaborative Care Team in Action”), the key members of the collaborative care (CC) team and the specific tasks for the primary care provider (PCP) and behavioral health provider (BHP) were identified. This chapter will explore the roles of the psychiatric consultant, which consist of a diverse range of activities, including providing informal consultation, direct evaluation of patients, education, population-based care, and leadership. Psychiatrists are uniquely suited to lead teams in the primary care setting and ensure team cohesiveness, spanning the historically separate cultures of primary care and behavioral health that has proven to be one of the greatest challenges to effective implementation. Effective psychiatric consultation will be an asset to primary care organizations pursuing certification as patient-centered medical homes (PCMHs), with their emphasis on managing multiple chronic conditions, including behavioral health disorders.
