Pediatric primary care providers (PCPs) have a long history of promoting integrated care with a goal of providing a comprehensive care environment for families. The term medical home was, in fact, first utilized by the American Academy of Pediatrics back in 1967 and subsequently evolved as a concept over the following decades to the point where the patient-centered health care home is now widely embraced by all of primary care medicine (Sia et al. 2004). The passage of the Patient Protection and Affordable Care Act in 2010 has accelerated the adoption of integrated models of pediatric primary care as a strategy to improve the health of populations and the quality and affordability of health care services.
