In this chapter, we intend to provide mental health practitioners with updated, evidence-based guidelines in the management of the most common modifiable risk factors for cardiovascular disease (CVD) (obesity, diabetes, dyslipidemia, hypertension, tobacco use disorders), the leading cause of mortality for adults with severe and persistent mental illness (Colton and Manderscheid 2006; Crump et al. 2013a, 2013b). The following sections provide a framework for screening, care management, and therapeutic risk reduction in this vulnerable population. By focusing on pragmatic approaches that may be pursued within the psychiatric scope of practice, we aim to equip psychiatrists with the tools necessary to understand and work within an integrated primary care system. A single case will be used to highlight various clinical points over the course of the chapter in each of the sections and will be referred to at various points throughout the chapter.
