The formulation is the linchpin of treatment planning. More than a summary of the case, it comprises the major elements of the patient’s history, current problems, and signs and symptoms synthesized into a set of ideas from which treatment decisions can emerge. At its simplest level, a formulation explains why the patient has the problems that brought him or her to the therapist.
