Principles of Effective Integrated Mental Health Care

Patient-Centered Care

• Team-based care relies on effective collaboration between primary care providers and mental health providers.

Population-Based Care

• Mental health patients are tracked in a registry: no one "falls through the cracks."

Measurement-Based Treatment to Target

• Measurable treatment goals and outcomes are defined and tracked for each patient.
• Treatments are changed until the clinical goals are achieved.

Evidence-Based Care

• Treatments used are evidence based.

Pay for Performance

• The delivery system is accountable and reimbursed according to quality of care, clinical outcomes, and patient satisfaction, not just the volume of care provided.
